Patient Forms As a new patient, please help us get to know you and your health concerns by printing and completing the following patient forms: - New Patient Information Form-1.pdf - Release of Medical Information - Rochester Rhio - Patient Consent Form Insurance We participate with most major insurance plans and Medicare. Please contact the office prior to your first visit just to be sure that your insurance is accepted by our practice. Patient Financial Responsibility We expect your co-payment to be paid at the time of service. We accept cash, check & charge. Please notify the office of any changes you may have (demographic information or insurance). Prescription Renewal requests will be filled Monday-Friday during regular business hours with at least 24 hour notice. Renewal of certain medications will require office visits to monitor progress.
